Ectropion is more common in the elderly and usually affects only the lower eyelid. With severe ectropion, the entire length of the eyelid is rotated. With less severe ectropion, a single segment of the eyelid moves away from the eye.

Usually when you blink, your eyelids will evenly distribute the tears over your eyes, keeping the surfaces of the eyes lubricated. These tears flow into small openings in the inner part of your eyelids (puncta).

When you have ectropion, your lower eyelid will separate from your eye and the tears will not flow properly into the puncta. The resulting signs and symptoms can include:

Watery eyes (excessive tearing). Without adequate drainage, your tears can pool and run down your eyelids constantly.
Excessive drought. Ectropion can make your eyes dry, grainy, and sandy.

The reasons

Ectropion can be caused by:

Muscle weakness. As you age, the muscles under your eyes weaken and the tendons stretch. These muscles and tendons keep your eyelid stretched against your eye. As they weaken, your eyelid can sag.
Facial paralysis. Certain medical conditions like Bell’s palsy and certain types of tumors can paralyze the nerves and muscles of the face. Facial paralysis that affects the eyelid muscles can lead to ectropion.
Scars or previous surgeries. Skin damaged from burns or trauma such as a dog bite can affect the way your eyelid rests against your eye. Risk factors

Factors that increase your risk of developing ectropion include:

Age. The most common cause of ectropion is the weakening of muscle tissue associated with aging.
Previous eye surgeries. People who have had eyelid surgery are at higher risk of developing ectropion later.
Cancer, burns, or previous trauma. If you’ve had skin cancer patches on your face, face burns, or trauma, you are at greater risk of developing ectropion.
