A testicle without sagging (cryptorchidism) is a testicle that, prior to birth, did not move into the correct position in the skin sac that hangs under the penis (scrotum). Not seeing or feeling a testicle where you would expect it to be in the scrotum is the main sign of a testicle without sagging.
The testes form in the abdomen during fetal development. During the last two months of normal fetal development, the testicles gradually descend from the abdomen through a tubular passage through the groin (groin duct) into the scrotum. The reasons
The exact cause of a scrotum is not known. A combination of genetics, maternal health, and other environmental factors can disrupt the hormones, physical changes, and nerve activity that affect testicular development.
Risk factors
Factors that can increase the risk of testicular loss in a newborn include:
- Low birth weight
- Premature birth
- Family history of testicles with no sagging or other genital development problems
- Conditions of the fetus that may limit growth, such as Down’s syndrome or an abdominal wall abnormality

Complications
In order for the testicles to develop and function normally, they need to be slightly cooler than normal body temperature. The scrotum provides this cooler environment. Complications of a testicle that are not meant to be include:
